CUHSM has successfully submitted MACRA compliant quality submission formats.

To expedite your CMS 2017 MACRA Quality Submission, please complete these steps:

1) You must register by completing two application forms:

(a) Complete the QPP form HERE

(b) Complete the MACRA Questionnaire HERE

2) Email us at clientservices@cuhsm.org
        Subject: Registration for 2017 MACRA Beta program
        Email must include name, phone # and website address.
